Michigan’s Peace of Mind Registry; A Convenient Place to Store Your Health Care Power of Attorney On-Line

The State of Michigan has created a convenient on-line tool for individuals to store their Health Care Powers of Attorney (also known as a Patient Advocate Form). Michigan’s on-line statewide registry is called “Peace of Mind” and can be accessed online at www.mipeaceofmind.org, or you can call 1-800-482-4881.

Once you have stored your Health Care Power of Attorney with the Peace of Mind Registry, the State of Michigan will send you a wallet card with instructions on how you and your care givers can access it on-line. Eventually, it is the goal of the Peace of Mind Registry to allow your health care providers to access your Health Care Power of Attorney without any additional effort by you, but the Registry does not have that functionality at this time.  Please note that you can revoke your Health Care Power of Attorney through the Registry at any time, and there is no cost to use the Registry.  If you would rather use the U.S. mail to send your Health Care Power of Attorney to be recorded on the Registry rather than the website, or if you need assistance with your registration, you can send your Health Care Power of Attorney to Gift of Life Michigan, 3861 Research Park Drive, Ann Arbor, MI 48103, or contact Michigan Peace of Mind for assistance using the contact information provided at the website.
